Deacon's Fund Application
Restoration Church will accept requests for assistance from members of Restoration Church. Requests will be considered as one-time, emergency needs (i.e.: eviction notice, medication, utilities shut-off notice, food assistance) within a 12 month period.
Strict guidelines are in place regarding amounts provided. These guidelines are non-negotiable.
Please complete this form to the best of your ability. Requests are addressed Monday-Thursday, excluding holidays. You will be contacted within 24-48 hours during the designated days. The Deacons' Fund Team does NOT have an office at Restoration Church and cannot be contacted via the church.
Please note that submission of this form does not guarantee approval. By filing out this application for assistance, you grant Restoration Church Deacons' Fund Team permission to verify any and all information as contained in your request.
If you are approved, you will be required to come to the church with all appropriate documentation (i.e.: photo ID, rent receipts, lease, utility receipts, shut off notices, etc.). Without this information, you will NOT receive assistance. Please note that NO checks will be mailed directly from Restoration Church.
Checks need to be picked up by the applicant. All checks issued will be written directly to the creditor and NOT to the applicant. It will be your responsibility to bring the payment to the appropriate person for payment.
